Sign Up Policies & Procedures

Prior to class attendance for group, private or semi-private lessons, there are several forms that need to be printed, signed and returned to West Hawaii Water and Safety Instructions prior to class start date. 
These include the Policies & Procedures form (which covers our cancellation policy and payment adjustments), the Liability Release form and the Photo Release form.
